Patient Policies

Patient Policies
Effective Date: 01/01/23

The following policies are intended to provide clarity around consultations, appointments, memberships, payments, telehealth, and communication.

Consultations and Appointments

Submitting a request for consultation or booking through the website does not guarantee acceptance as a patient. Oakwood Precision Medicine reserves the right to determine clinical fit, service availability, and scheduling eligibility.

Cancellation and Rescheduling Policy

We ask that cancellations or rescheduling requests be made at least 24–48 hours in advance. Missed appointments or late cancellations may be subject to a fee.

You can adjust this to your preference:

  • 24 hours for a more approachable model

  • 48 hours for a more premium concierge model

Payment Policy

Payment is due at the time of service unless otherwise stated in writing. Certain services, diagnostics, testing, or specialty programs may require prepayment or a deposit.

Memberships and Programs

If Oakwood offers memberships or ongoing wellness programs:

  • Membership fees are billed according to the plan selected

  • Benefits apply only during the active membership period

  • Membership terms, inclusions, exclusions, and pricing may change with notice

  • Memberships may be non-refundable except where required by law or expressly stated otherwise

Refund Policy

Fees for completed services, consultations, and medical provider time are generally non-refundable. Refunds for prepaid packages, memberships, products, or unused services, if any, are governed by the specific terms provided at enrollment.

Telehealth Policy

If telehealth services are offered, patients acknowledge that:

  • telehealth may involve electronic communication

  • there are limitations compared with in-person care

  • not all services are appropriate for telehealth

  • telehealth availability may depend on provider licensure and patient location

Formal consent may be required before receiving telehealth services.

Communication Consent

By submitting your contact information, you consent to Oakwood contacting you by phone, email, or text regarding appointments, services, scheduling, and relevant practice communications. Message and data rates may apply. You may opt out of promotional communications at any time.

Not Insurance-Based Care

If applicable, add:
Oakwood Precision Medicine operates as a private-pay practice and may not accept insurance reimbursement for all services. Patients are responsible for understanding their own coverage and reimbursement eligibility.

Patient Responsibility

Patients are responsible for providing accurate health information, following clinical recommendations, reviewing informed consents, and asking questions before beginning any treatment plan.
